Transition to Independence Process
What is TIP? TIP is an evidence-based practice that helps youth and young adults become independent and achieve their goals as they transition to adulthood. The TIP program helps them determine what is important across several life areas including: living situation, education, employment, community life, physical & mental health, positive relationships, and good communication.
How Does TIP Work?
The TIP Model is a strength-based, youth-driven framework that was developed for working with youth and young adults (14-29 years old) with emotional/behavioral difficulties. Transition to Independence Process (TIP) model website: http://NNYT.TIPstars.org

Services are youth-friendly and provided in the community so that individuals learn to navigate their world. The TIP Program has been proven effective in helping young adults obtain independent living, reduce court involvement, and increase management of mental health symptoms.

Treatment emphasizes youth strengths and interests, focuses on the future, enhances problem solving and decision making, promotes healthy relationships, and teaches coping skills and life skills. Staff work with young adults to accomplish simple tasks, such as taking the bus or cleaning their home, that can feel overwhelming to a young person who is experiencing serious mental health symptoms. The program takes a very hands-on approach focusing on learning by doing.

Criteria & Referral info for Hamilton County TIP services

Potential client must range in age from 16-22

Must meet DSM-V eligibility for emotional or behavioral difficulties.

​Must have involvement in multiple systems, such as such as behavioral health, child welfare, criminal justice (adult & juvenile), developmental disabilities services, educational or vocational settings.
Agencies may submit the referral form and a recent DAF (completed within the past year) to: bflorimonte@gcbhs.com

Criteria & Referral info for Clermont County TIP services

Potential client must range in age from 14-24

Experiencing emotional or behavioral difficulties.

​Must be a resident of Clermont County.​

No referral or appointment necessary.
For the assessment of services:
-
a parent/guardian and youth must both be present for the entire appointment
-
custody paperwork is required unless both parents are present
-
for multiple children, each will go through the process individually
