Homeless Services and PATH
GCB has several programs designed to respond to the needs of individuals who are homeless and have severe mental illness. The PATH Team provides street outreach and works with area shelters and homeless services to engage homeless individuals and connect them to services. PATH uses unique staffing such as a Peer Worker, a Forensic Worker and a Veteran Worker to better engage and relate to special populations. The Homeless ACT Team uses a Housing First approach to serve individuals with chronic mental illness and chronic homelessness by providing intensive, team-based wrap-around services to give this population the support, treatment and hope that they need to thrive. |
