Forensic Assertive Community Treatment (FACT)
The FACT team provides intensive community support to individuals who are mentally ill and are being released from prison on parole or post release control. The team collaborates closely with the state prison system to link program participants to services immediately upon release and uses a unique approach to serving this population in the community by integrating a dedicated parole officer onto the team. |
