Case Management
Case Management refers to coordinating the overall care of our clients and to improve and restore their mental health and overall functioning in the community. Case managers may assist with Social Security benefits, medical benefits, housing and crisis management. They may refer clients to job training and placement, day programs, client support groups, family support and education groups, and specialized services. Our case managers collaborate with family members, psychiatrists, medical professionals and other service providers to ensure optimal care for clients. |
