Case Manager
Full time position responsible for providing Community Psychiatric Support Treatment (CPST) services to severely and persistently mentally ill clients.  Establishes therapeutic relationships, assesses complex problems, selects problem-solving interventions, and helps clients to function effectively.   Assesses each client’s strengths and limitations and the mental health, financial, and institutional resources available.  Collects, interprets, and analyzes information about a client’s medical, voca­tional, social, educational, housing, financial, transportation, childcare, and other basic needs.  In conjunction with a supervisor and team psychiatrist, assesses the client’s mental health and basic needs, determines the appropriate level of care and support, determines the availability of resources, and develops an Individual Recovery Plan.  Implements and monitors the Individual Recovery Plan.  Coordinates access to the recommended medical treatment services, vocational assistance, housing assistance, and other targeted services.  Monitors the treatment progress of clients, reassesses client status and the effectiveness of interventions, revises the Individual Recovery Plan, and advocates for clients.  Coordinates, conducts or participates in assessments; provides crisis assistance services; delegates recovery plan tasks to appropriate service providers; and monitors overall service delivery.  Master's degree in mental health, vocational training, education, social work, counseling or other field related to the mentally ill; or a Bachelor's degree in one of the specified fields plus two years of education, training, or experience in one of the specified fields.  Position requires the use of a personal mobile phone.  Position requires frequent driving of own personal vehicle.  A valid driver’s license, vehicle, and insurable driving record are required.  

Case Manager-Transitional Youth Services (see description above)
Full time position responsible for providing Community Psychiatric Support Treatment (CPST) services to severely and persistently mentally ill clients who are transition aged youth who are 16-22 years old.

Case Manager-Homeless Transitional Services

Full time position responsible for providing case management to a caseload of clients who have a history of severe and persistent mental illness.  Evaluate the need for medical, social, educational, voca­tional and other services essential to meeting basic human needs and assist the client to gain access to these.  Coordinate assessments, treatment planning and crisis assistance services; provide linkage and training for the client in the use of basic community resources; and monitor overall service delivery.  Participate in on-call system. Bachelor’s degree and two years experience in Human Services or related field required.  Excellent communication, documentation, time management, coordination and collaboration skills; ability to mobilize community resources and to coordinate in-home and transitional services; experience in crisis intervention; basic knowledge of developing service plans; and basic computer skills are required.  Position requires use of personal mobile phone.  Position requires frequent driving of own vehicle.  Valid driver’s license, vehicle and insurable driving record are required.

Job Coach
Full time position in One-Stop Employment Program to assist severely, mentally disabled adults to maintain employment.  Duties include job/task analysis, orientation and job training, personal/work adjustment and other on-site/off site training.  Bachelor’s degree with preference for Human Services or Business plus two years of related experience required or Associate’s degree with preference for Human Services or Business plus four years of related experience required.  Knowledge of task/job analysis, behavior management, work/personal adjustment management and methodology, and private industry standards; excellent communication, organizational and time management skills; and basic computer knowledge required.  Position requires use of personal mobile phone. Position requires frequent driving of own vehicle.  Valid driver’s license, vehicle and insurable driving record are required.

Peer Job Coach
Part time position (15 hours) responsible for employment site supportive services such as, orientation and job training, and other on-site and off-site training and support as required to assist (SMD) severely mentally disabled clients to maintain employment.  Individual must be a former or current primary mental health consumer with demonstrated ability to respectfully motivate other consumers to act on their own behalf and to participate in activities. Associate’s degree preferred and/or two years of related experience required.  Ability to demonstrate peer-to-peer skills in direct service provision and to work within a team environment and to interact with employers appropriately on behalf of the client receiving services.  Position requires frequent driving of own vehicle.  Valid driver’s license, vehicle and insurable driving record are required.

Supported Housing Specialist
Full time position, may include some evening and/or weekend hours, to participate as member of outreach team providing skills training and intensive support to SMD clients living in the community.  Associate’s degree in Human Services plus two years of related experience, or total of 4 years combined of education and related experience required.  Excellent oral and written communication, documentation, and time management skills; ability to work with SMD population; basic computer and keyboard knowledge; ability to be self-directed and to meet outcomes as established for program, ability to coordinate services for program’s clients.  Position requires use of personal mobile phone.  Position requires frequent driving of own vehicle.  Valid driver’s license, vehicle and insurable driving record are required.

 

Transitional Support Specialist

Full time position to perform intake functions and provide intensive support services to SMD clients living in or transitioning to the community.  Services to include medication training, independent living skills training, interpersonal skills development, assistance with managing psychiatric symptoms, as well as, provision of ongoing intensive support to assist clients in resolving potential crisis situations. Bachelor’s degree in human services field and two years experience in a human services agency working in a community-based mental health program working intensively with SMD population; or equivalent combination of education and experience in human services agency; total combination of six years education and experience required.  Excellent time management, communication and documentation skills and ability to be flexible with work schedule required.  Position requires use of personal mobile phone.  Position requires frequent driving of own vehicle. Valid driver’s license, vehicle and insurable driving record are required.

Full time position responsible for the administrative and clinical management of assigned case management team.  Provides coordination, leadership, and supervision to the case managers.  Manages the daily operation of team supporting organization goals and philosophy.  Master’s degree in human services or related field plus 2 years post-graduate clinical experience and  licensure by the Ohio Counselor and Social Worker Board (LISW, LPCC, or LPC) or otherwise credentialed to supervise case management services (M.S.N., licensed psychologist).  May consider applicants who are within 1 year of completion of credentials.  Demonstrated leadership and supervision skills are required.  Position may require use of personal mobile phone.  Position requires frequent driving of own vehicle.  Valid driver’s license, vehicle and insurable driving record are required.
